Sign Up · Step 3 of 4 — the caregiver form

Get paid as a caregiver

Care for a family member — or, in some cases, others — as a Delmarva Family Care employee, paid regular W-2 wages for authorized, verified hours. Signing up holds your place on our interest list.

Now forming in Delaware

Delmarva Family Care is now forming in Delaware. We are not yet a licensed Delaware Personal Assistance Services Agency or an enrolled Delaware Medicaid provider, and we are not yet providing care services. Signing up is free, holds your place on our interest list, and is not a promise of employment or services. When licensing and enrollment are complete, we will contact families and caregivers in the order they signed up.

Before you fill this in — what the job requires

This short form only holds your place; it never asks for a Social Security number or a date of birth. Being hired has real prerequisites under Delaware law — age 18+, a fingerprint background check, drug screening with results before your first visit, orientation and a competency test, and a health file. Read the complete caregiver requirements on the Sign Up page — each one explained, with its government source. You can fill in this form first and read them after; your place is held either way.

After you submit

You’ll land on a confirmation page. While you wait for enrollment to open, the packet explains the whole program — including exactly how caregiver pay works.
